ABSTRACT Introduction: Kidney problems may be due to low birth weight alone or may occur in association with other conditions. The objective this study was to evaluate the association between maternal and birth characteristics, anthropometric measurements, and kidney function deficit in low birth weight infants. Methods: Cross-sectional study with children who were born weighing < 2500 grams and were under outpatient follow-up. Maternal factors investigated were prenatal care and presence of hypertension, diabetes, and infection during pregnancy. The children's variables were sex, gestational age, birth weight, Apgar score, use of nephrotoxic medications, age, body weight at the time of evaluation, height, and serum creatinine and cystatin C dosages. The glomerular filtration rate (GFR) was estimated with the combined Zapittelli equation. Multivariate logistic regression model was used for identification of associated factors, with renal function deficit (GFR < 60 mL/min/1.73 m2) as the dependent variable. Results: Of the 154 children evaluated, 34.42% had kidney function deficit. Most of them had a gestational age > 32 weeks (56.6%), a mean birth weight of 1439.7 grams, and mean estimated GFR of 46.9 ± 9.3 mL/min/1.73 m2. There was a significant association of GFR < 60 mL/min/1.73 m2 with children's current weight and use of nephrotoxic drugs. Discussion: Children born with low birth weight had a high prevalence of kidney function deficit and current normal weight was a protective factor while the use of nephrotoxic drugs during perinatal period increased the chance of kidney deficit. These findings reinforce the need to evaluate the kidney function in these children, especially those who use nephrotoxic drugs.


RESUMO Introdução: Problemas renais podem ser devido apenas ao baixo peso ao nascer ou podem ocorrer em associação com outras condições. O objetivo deste estudo foi avaliar a associação entre características maternas e de nascimento, medidas antropométricas e déficit da função renal em bebês de baixo peso ao nascer. Métodos: Estudo transversal com crianças que nasceram com peso < 2500 gramas e estavam sob acompanhamento ambulatorial. Os fatores maternos investigados foram cuidados pré-natal e presença de hipertensão, diabetes e infecção durante a gravidez. As variáveis das crianças foram sexo, idade gestacional, peso ao nascer, índice Apgar, uso de medicamentos nefrotóxicos, idade, peso corporal no momento da avaliação, altura e dosagens séricas de creatinina e cistatina C. A taxa de filtração glomerular (TFG) foi estimada com a equação combinada de Zapittelli. Utilizou-se um modelo de regressão logística multivariada para identificação de fatores associados, com déficit da função renal (TFG < 60 mL/min/1,73 m2) como variável dependente. Resultados: Das 154 crianças avaliadas, 34,42% apresentaram déficit da função renal. A maioria tinha idade gestacional > 32 semanas (56,6%), peso médio ao nascer de 1439,7 gramas, e TFG média estimada de 46,9 ± 9,3 mL/min/1,73 m2. Houve uma associação significativa da TFG < 60 mL/min/1,73 m2 com o peso atual das crianças e o uso de medicamentos nefrotóxicos. Discussão: Crianças nascidas com baixo peso apresentaram alta prevalência de déficit da função renal e o peso atual normal foi um fator de proteção, enquanto o uso de medicamentos nefrotóxicos durante o período perinatal aumentou a chance de déficit renal. Estes achados reforçam a necessidade de avaliar a função renal destas crianças, especialmente aquelas que usam medicamentos nefrotóxicos.

OBJECTIVES: The prevalence of obesity in the population has increased and excess body adiposity is one of the main nutritional disorders in patients with chronic kidney disease (CKD) on hemodialysis (HD). The objective of this study was to develop equations using anthropometric measurements to predict the total and abdominal body adiposity of patients with CKD on HD. METHODS: This is a cross-sectional study evaluating 323 patients with CKD on HD in city in northeastern Brazil. Measurements and anthropometric indicators were correlated with percentage of body fat (%BF) and visceral fat (VF, in kg) measured by dual energy X-ray absorptiometry. Multiple linear regression models based on different combinations of anthropometric measurements were adjusted to develop the equations, with subsequent cross-validation. RESULTS: Of the 323 patients, 62.2% were male and 46.5% were aged between 40 and 59 years. The equation selected to estimate %BF included weight, height, waist and hip circumferences, and triceps and suprailiac skin folds, presenting high predictive capacity (R2 = 0.771). The equation selected to estimate VF included weight, height, waist circumference, hip circumference, and sum of skin folds (R22 = 0.796). CONCLUSIONS: The proposed equations efficiently predicted the %BF and VF (kg) of patients with CKD on HD, thereby serving as viable indicators in clinical practice.

The objective was to perform a spatial analysis of the hospital mortality rate (HMR) due to severe acute respiratory syndrome (SARS) attributed to COVID-19 among children and adolescents in Brazil from 2020 to 2021. A cluster method was used to group federal units (FUs) based on HMR. In 2020, clusters with high HMRs were formed by north/northeast FUs. In 2021, there was a reduction in HMR. Clusters with higher rates remained in the N/NE region. Regional differences were observed in the HMR. The findings may reflect social inequalities and access to hospital care, especially in the under 1-year-old age group due to the severity of the disease in this group.

Resumo Objetivou-se realizar uma análise espacial da taxa de mortalidade hospitalar (TMH) por síndrome respiratória aguda grave (SRAG) atribuída à COVID-19 em crianças e adolescentes no Brasil no período de 2020 a 2021. Utilizou-se o método de cluster para agrupar as unidades federativas (UFs) com base na TMH. Em 2020, clusters com altas TMHs foram formados por UFs Norte/Nordeste. Em 2021, houve redução na TMH. Os clusters com maiores taxas permaneceram na região N/NE. Diferenças regionais foram observadas nas TMHs. Os achados podem refletir as desigualdades sociais e o acesso à atenção hospitalar, principalmente na faixa etária de menores de 1 ano pela gravidade da doença neste grupo.

OBJECTIVE: To analyze the clinical and sociodemographic characteristics and survival of individuals with severe acute respiratory syndrome due to COVID-19 according to the COVID-19 vaccination schedule, Brazil, 2021-2022. METHODS: This was a cohort study based on data from the Influenza Epidemiological Surveillance Information System; the Kaplan-Meier and Survival Tree methods were used to analyze survival. RESULTS: Among the 559,866 hospitalized cases, a higher proportion of vaccinated individuals was found among female (15.0%), elderly people aged ≥ 80 (34.5%), people from the Southeast region (15.7%), those who did not undergo respiratory support (21.2%) and those who did progress to death (15.2%); the survival curve showed that risk of death for unvaccinated individuals was higher in all age groups (p-value < 0.001); elderly people aged ≥ 80, who did not undergo mechanical ventilation and who had a booster dose had lower risk when compared to their peers who had two doses or were unvaccinated (hazard ratio = 0.64; 95%CI 0.62;0.67). CONCLUSION: Lowest risk of death was found in vaccinated individuals, especially those who had two doses or a booster dose as well. MAIN RESULTS: Prevalence was found to be high among unvaccinated individuals. Risk of death was lower among those vaccinated with a booster dose, compared to those not vaccinated, in all age groups analyzed. IMPLICATIONS FOR SERVICES: The number of hospitalizations of unvaccinated individuals with severe acute respiratory syndrome was high, which increases the demand for health services to care for these individuals. PERSPECTIVES: It is necessary to promote widespread vaccination of the entire population of Brazil, in addition to the regular provision of booster doses for the different population groups.

BACKGROUND: Traditional methods for assessing individual energy consumption often involve lengthy and intricate procedures. This study aims to introduce an Energy Consumption Estimation Scale, utilizing Item Response Theory (IRT) for adolescents aged 18-19 years. METHODS: This psychometric investigation applies IRT to 93 items extracted from a validated food frequency questionnaire. The study encompasses a representative sample of 2515 adolescents from the São Luís birth cohort in Brazil. The latent trait, energy intake, is derived using IRT and subsequently validated through hierarchical multiple linear regression modeling. Significance was established at p < 0.05. RESULTS: A Samejima's model was successfully fitted (CFI and TLI > 0.9 and RMSEA < 0.08), effectively capturing variations across all energy consumption levels. Factors associated with the latent trait demonstrate consistent behavioral patterns. Adolescents with higher energy intake exhibited increased consumption of dairy products, artificially sweetened beverages, and seasonal fruits and vegetables. CONCLUSIONS: The proposed Energy Consumption Estimation Scale demonstrates a reliable measurement of energy intake and serves as a practical and concise alternative for assessing energy consumption among adolescents. These findings suggest the potential for adapting similar models for different age groups and incorporating diverse food items based on the obtained results.

There seems to be a consensus that adolescents worldwide are not getting enough sleep. This study aimed to evaluate the psychometric properties of the Epworth Sleepiness Scale (ESS) in adolescents using the item response theory. A psychometric study was conducted with 2206 adolescents aged 18 and 19 years in the city of São Luís, Maranhão, Brazil. The dimensionality of the ESS was assessed by principal component analysis. A Samejima's graded response model (SGRM) was fitted to it. The findings of this study showed a good internal consistency and the unidimensionality of the ESS. Considering the latent trait continuum, we obtained three levels, with anchor items. For the item 'Sitting still in a public place', the adolescents presented a small possibility of dozing in level 1, and a medium and great possibility in level 2. The item 'Sitting around talking to someone' presented small, medium, and great possibilities of dozing in level 3. The ESS with five items showed satisfactory psychometric properties. In addition, the results from the SGRM showed that adolescents with high levels of sleepiness are likely to nod off or sleep sitting up while talking to someone. This study allows us to understand excessive daytime sleepiness in adolescents.

Obesity is a recognized risk factor for the development of cardiometabolic outcomes. Therefore, it is essential to evaluate anthropometric and body composition indicators used for its diagnosis. This study aimed to assess the diagnostic performance of body fat percentage (BF%), fat mass index (FMI) and body mass index (BMI) for detecting cardiometabolic outcomes in adults. A cross-sectional study was conducted involving adults at 30 years of age from Pelotas, RS (n = 3517) and at 37-39 years from Ribeirão Preto, SP (n = 1696). Receiver operating characteristic (ROC) curves were used to determine the cut-off points for predicting cardiometabolic risk factors, including altered blood pressure, blood glucose, triglycerides, total cholesterol, low-density lipoprotein cholesterol (LDL-c), high-density lipoprotein cholesterol (HDDL-c), C-reactive protein and glycated hemoglobin. The cut-off points of BF% ranged from 25.2 to 27.8 in men and from 37.4 to 39.7 in women at 30 years, and from 26.1 to 27.8 in men and from 38.5 to 42.2 in women at 37-39 years. For FMI (kg/m2), the cut-off points ranged from 6.3 to 7.5 in men and from 9.5 to 10.8 in women at 30 years, and from 7.3 to 7.8 in men and from 10.2 to 12.2 in women at 37-39 years. The BMI cut-off points (kg/m2) ranged from 26.3 to 27.3 in men and from 25.4 to 27.2 in women at 30 years, and from 28.3 to 29.0 in men and from 27.2 to 29.6 in women at 37-39 years. The areas under the curve were similar for the three indicators, ranging from 0.523 to 0.746. BMI showed a performance similar to that of the body fat-based indicators in identifying cardiometabolic outcomes. The cut-off points of the three indicators showed acceptable discriminatory power in subjects with cardiometabolic risk factors.

Beriberi is the clinical manifestation of severe and prolonged thiamine (vitamin B1) deficiency. It is a neglected disease that affects low-income populations facing food and nutrition insecurity. The aim of this study was to compare cases of beriberi among indigenous and non-indigenous people in Brazil. We conducted a cross-sectional study using data on cases of beriberi during the period July 2013-September 2018 derived from beriberi notification forms available on the FormSUS platform. Cases in indigenous and non-indigenous patients were compared using the chi-squared test or Fisher's exact test, adopting a significance level of 0.05. A total of 414 cases of beriberi were reported in the country during the study period, 210 of which (50.7%) were among indigenous people. Alcohol consumption was reported by 58.1% of the indigenous patients and 71.6% of the non-indigenous patients (p = 0.004); 71.0% of the indigenous patients reported that they consumed caxiri, a traditional alcoholic drink. Daily physical exertion was reported by 76.1% of the indigenous patients and 40.2% of the non-indigenous patients (p < 0.001). It is concluded that beriberi disproportionately affects indigenous people and is associated with alcohol consumption and physical exertion.


O beribéri é a manifestação clínica da deficiência grave e prolongada de tiamina (vitamina B1). Doença negligenciada que acomete a população de baixa renda, em situação de insegurança alimentar e nutricional. O objetivo do estudo foi comparar casos de beribéri em indígenas com casos em não indígenas no Brasil. Trata-se de estudo transversal de casos notificados de beribéri no período de 2013 a 2018, no formulário do SUS (FormSUS) do Ministério da Saúde. Foram comparados os casos em indígenas e em não indígenas pelo teste qui-quadrado ou teste exato de Fisher com nível de significância de p < 0,05. No período estudado foram notificados no país 414 casos de beribéri, sendo 210 (50,7%) indígenas. Referiram consumo de bebidas alcoólicas 58,1% dos indígenas e 71,6% dos não-indígenas (p = 0,004); adicionalmente, 71,0% dos indígenas consumiam caxiri (bebida alcoólica tradicional fermentada). Relataram fazer esforço físico diário 76,1% dos indígenas e 40,2% dos não-indígenas (p <0 ,001). Conclui-se que o beribéri no país acomete mais indígenas e está relacionado ao consumo de álcool e ao esforço físico.

RESUMO

[RESUMO]. Objetivo. Comparar as taxas de mortalidade hospitalar (TMH) por síndrome respiratória aguda grave (SRAG) associada à covid-19 registradas em regiões metropolitanas e no interior do Brasil em 2020 e 2021. Método. Trata-se de um estudo ecológico com dados públicos disponíveis no OpenDataSUS. As informações foram acessadas em maio de 2022. Consideraram-se as seguintes variáveis: idade, sexo, internação hospi- talar, presença de fator de risco, internação em UTI, uso de suporte ventilatório e classificação final na ficha de registro individual de casos de SRAG por covid-19. Os casos e óbitos foram estratificados em cinco faixas etárias (0-19 anos, 20-39 anos, 40-59 anos, 60-79 anos e ≥80 anos) e por localização do município de resi- dência (região metropolitana ou interior). A TMH teve como numerador o número absoluto de óbitos por SRAG associada à covid-19; e, como denominador, o número absoluto de casos de SRAG por covid-19 segundo ano de ocorrência, residência em região metropolitana ou interior, faixa etária, sexo, internação hospitalar, presença de fator de risco, internação em unidade de terapia intensiva (UTI) e uso de suporte ventilatório. Resultados. Verificou-se aumento significativo da TMH por SRAG associada à covid-19 em 2021 em todos os grupos etários, exceto 0-19 anos e ≥80 anos, assim como entre indivíduos internados em UTI e que utilizaram suporte ventilatório invasivo, tanto nas regiões metropolitanas quanto no interior. Conclusões. Houve piora do cenário epidemiológico em 2021 com o aumento da TMH, mas não foram iden- tificadas diferenças entre as regiões metropolitanas e o interior do país.


[ABSTRACT]. Objective. To compare hospital mortality rates (HMR) due to severe acute respiratory syndrome (SARS) asso- ciated with COVID-19 recorded in metropolitan areas and other regions (interior) of Brazil in 2020 and 2021. Method. This ecological study used public data available on OpenDataSUS. The information was accessed in May 2022. The following variables were considered: age, sex, hospitalization, presence of a risk factor, ICU stay, use of ventilatory support, and final classification in the individual registration form of SARS cases due to COVID-19. Cases and deaths were stratified into five age groups (0-19 years, 20-39 years, 40-59 years, 60-79 years, and ≥80 years) and by place of residence (metropolitan area or interior). The HMR had as numerator the absolute number of deaths by SARS associated with covid-19; and, as a denominator, the absolute number of cases of SARS due to covid-19 according to the year of occurrence, area of residence, age bracket, sex, hospitalization, presence of a risk factor, ICU admission, and use of ventilatory support. Results. There was a significant increase in HMR due to SARS associated with COVID-19 in 2021 in all age groups, except 0-19 years and ≥80 years, as well as among individuals admitted to an ICU and who used invasive ventilatory support, both in metropolitan areas as well as in the interior. Conclusions. There was a worsening of the epidemiological scenario in 2021 with an increase in HMR. Howe- ver, no differences were identified between the metropolitan regions and the interior of the country.


[RESUMEN]. Objetivo. Comparar las tasas de mortalidad hospitalaria por el síndrome respiratorio agudo grave relacionado con la COVID-19 registradas en las regiones metropolitanas y el interior de Brasil en el período 2020-2021. Método. Se realizó un estudio ecológico con datos públicos disponibles en el sistema OpenDataSUS. La información se consultó en mayo del 2022. Se tomaron en cuenta las siguientes variables: edad, sexo, hos- pitalización, presencia de factores de riesgo, ingreso en la unidad de cuidados intensivos, uso de apoyo ventilatorio y clasificación final en la hoja de registro individual de casos del síndrome respiratorio agudo grave por COVID-19. Los casos y las defunciones se estratificaron en cinco grupos etarios (0-19 años, 20-39 años, 40-59 años, 60-79 años y ≥80 años) y por ubicación del municipio de residencia (región metropolitana o interior). El numerador de la tasa de mortalidad hospitalaria fue el número absoluto de defunciones por el síndrome respiratorio agudo grave relacionado con la COVID-19, y el denominador, el número absoluto de casos del mismo síndrome relacionado con la COVID-19 según el año de aparición, la residencia en una región metropolitana o en el interior, el grupo etario, el sexo, la hospitalización, la presencia de factores de riesgo, el ingreso en la unidad de cuidados intensivos y el uso de apoyo ventilatorio. Resultados. Se comprobó un aumento significativo de la tasa de mortalidad hospitalaria por el síndrome respiratorio agudo grave relacionado con la COVID-19 en el 2021 en todos los grupos etarios, excepto en los grupos de 0-19 años y ≥80 años, así como entre las personas internadas en la unidad de cuidados intensivos que recibieron apoyo respiratorio invasivo, tanto en las regiones metropolitanas como en el interior. Conclusiones. La situación epidemiológica empeoró en el 2021 con el aumento de la tasa de mortalidad hospitalaria, pero no se observaron diferencias entre las regiones metropolitanas y el interior del país.

Relataram fazer esforço físico diário 76,1% dos indígenas e 40,2% dos não-indígenas (p <0 ,001). Conclui-se que o beribéri no país acomete mais indígenas e está relacionado ao consumo de álcool e ao esforço físico.


Abstract Beriberi is the clinical manifestation of severe and prolonged thiamine (vitamin B1) deficiency. It is a neglected disease that affects low-income populations facing food and nutrition insecurity. The aim of this study was to compare cases of beriberi among indigenous and non-indigenous people in Brazil. We conducted a cross-sectional study using data on cases of beriberi during the period July 2013-September 2018 derived from beriberi notification forms available on the FormSUS platform. Cases in indigenous and non-indigenous patients were compared using the chi-squared test or Fisher's exact test, adopting a significance level of 0.05. A total of 414 cases of beriberi were reported in the country during the study period, 210 of which (50.7%) were among indigenous people. Alcohol consumption was reported by 58.1% of the indigenous patients and 71.6% of the non-indigenous patients (p = 0.004); 71.0% of the indigenous patients reported that they consumed caxiri, a traditional alcoholic drink. Daily physical exertion was reported by 76.1% of the indigenous patients and 40.2% of the non-indigenous patients (p < 0.001). It is concluded that beriberi disproportionately affects indigenous people and is associated with alcohol consumption and physical exertion.

Introdução: A tuberculose é um importante problema de saúde pública e atinge de maneira dessemelhante os espaços geográficos. Objetivo: analisar a distribuição espacial dos óbitos por tuberculose no Maranhão (Brasil) e sua associação com indicadores socioeconômicos. Método: Estudo ecológico dos óbitos por tuberculose no Maranhão, no período de 2010 a 2015, com unidade de análise, os municípios. Utilizou-se o índice de Moran global, correlograma e espalhamento do índice de Moran e para avaliar a associação espacial de óbitos por tuberculose com os indicadores socioeconômicos foram ajustados diferentes modelos espaciais condicionais autoregressivos. Resultados: No período, foram notificados 949 óbitos por tuberculose. Identificou-se uma forte dependência espacial na ocorrência dos óbitos. Por meio do modelo autoregressivo ajustado, constatou-se que aproximadamente 12% dos municípios possuem mais que 75% de chance de ocorrer um óbito por tuberculose e que o norte e a parte central do Estado são as regiões que concentram as maiores probabilidades de óbito. Conclusão: Houve forte dependência espacial na ocorrência de óbitos por TB, sendo esta afetada pelas taxas das áreas vizinhas. O índice de desenvolvimento humano municipal esteve negativamente associado com a taxa de óbitos por tuberculose e a renda média per capita apresentou associação positiva.

Objetivo: Analizar la distribución espacial de las muertes por tuberculosis en Maranhão (Brasil) y su asociación con indicadores socioeconómicos. Método: Estudio ecológico de las muertes por tuberculosis en Maranhão con municipios como unidad de análisis, entre 2010 y 2015. Se utilizaron el índice global de Moran, el correlograma y la dispersión del índice de Moran. Además, para evaluar la asociación espacial de las muertes por tuberculosis con los indicadores socioeconómicos, se ajustaron diferentes modelos espaciales autorregresivos condicionales. Resultados: Durante el período, se reportaron 949 muertes por tuberculosis. Se identificó una fuerte dependencia espacial en la ocurrencia de muertes. Utilizando el modelo espaciales autorregresivos condicionales ajustado, se encontró que, aproximadamente, el 12 % de los municipios tienen más del 75 % de probabilidad de muerte por tuberculosis. Además, que el norte y centro del estado son las regiones con mayor probabilidad de muerte por tuberculosis. Conclusión: Existió una fuerte dependencia espacial de la ocurrencia de muertes por tuberculosis, la cual fue afectada por las tasas en las áreas vecinas. El índice de desarrollo humano municipal se asoció negativamente con la tasa de muertes por tuberculosis y el ingreso per cápita promedio mostró una asociación positiva.


Introduction: Tuberculosis is an important public health problem that affects different geographical areas. Objective: To analyze the spatial distribution of deaths from tuberculosis in Maranhão (Brazil) and its association with socioeconomic indicators. Method: Ecological study of tuberculosis deaths in Maranhão, from 2010 to 2015, using municipalities as the unit of analysis. The global Moran index, the correlogram, and the spread of the Moran index were used, and to assess the spatial association of tuberculosis deaths with socioeconomic indicators, different conditional autoregressive spatial models (CAR) were adjusted. Results: During the chosen period, 949 deaths from tuberculosis were reported. A strong spatial dependence was identified in the occurrence of deaths. Using the adjusted conditional autoregressive spatial model, it was found that approximately 12 % of the municipalities have more than a 75 % chance of death from tuberculosis and that the north and central parts of the state have the highest probability of death from tuberculosis. Conclusion: There was a strong spatial dependence on the occurrence of deaths from tuberculosis, which was affected by the rates in neighboring areas. The municipal human development index was negatively associated with the rate of deaths from tuberculosis and the average per capita income showed a positive association.

OBJECTIVE: To determine neck circumference (NC) and waist-to-height ratio (WHtR) cut-off points as predictors of obesity and cardiovascular risk in adolescents. METHODS: Cross-sectional study developed with a subsample of 634 adolescents aged 18 and 19 years belonging to the third phase of the "RPS" cohort (Ribeirão Preto, Pelotas and São Luís) carried out in 2016. The area under the ROC curve (AUC) was identified to assess the predictive capacity of NC and WHtR in relation to the percentage of body fat (%BF), obtained by air displacement plethysmography (ADP), and the cardiovascular risk estimated by the Pathobiological Determinants of Atherosclerosis in Youth (PDAY). RESULTS: The prevalence of obesity by %BF was 7.6% in males and 39.4% in females (p-value <0.001), and the high PDAY risk was 13.8% and 10.9%, respectively. For males, NC cut-off point was 44.0 cm and the AUCs were 0.70 (95%CI 0.58-0.83) to predict obesity and 0.71 (95%CI 0.62-0.80) to predict high cardiovascular risk; for females, NC cut-off point was 40 cm and the AUCs were 0.75 (95%CI 0.69-0.80) and 0.63 (95%CI 0.53-0.73), respectively. WHtR cut-off point was 0.50 for both sexes; for males, the AUCs to predict obesity and high risk according to PDAY were 0.90 (95%CI 0.80-0.99) and 0.73 (95%CI 0.63-0.82), respectively; for females, they were 0.87 (95%CI 0.83-0.90) and 0.55 (95%CI 0.45-0.65), respectively. CONCLUSION: WHtR and NC are good discriminators to assess obesity and cardiovascular risk in adolescents, especially in males.

OBJECTIVE: To describe the clinical characteristics of cases of COVID-19 severe acute respiratory syndrome (SARS) in Brazilian newborns (NBs) in 2020 and 2021, recorded in the Influenza Epidemiological Surveillance Information System (Sistema de Informação da Vigilância Epidemiológica da Gripe - SIVEP-Gripe). METHODS: The variables analyzed were gender, race/skin color, hospitalization, intensive care unit (ICU) admission, use of ventilatory support, signs and symptoms (fever, cough, O2 saturation<95%, dyspnea, respiratory distress, diarrhea, and vomiting), progress (death or cure), risk factors/comorbidities. Categorical variables were expressed as absolute and relative frequencies. RESULTS: We found 1,649 records of COVID-19 SARS in NBs, with a predominance of multiracial babies in both years. The most frequent symptoms in 2020 and 2021 were, respectively: respiratory distress (67.0 and 69.7%), fever (46.3 and 46.2%), and cough (37.0 and 46.3%). In 2020, 30.5% of patients received invasive ventilatory support; in 2021, this number was 41.6%. In addition, more than 55% of cases required ICU admission, and over 16% died. CONCLUSION: We emphasize the high proportion of cases that required intensive care and progressed to death.

Introduction: high sodium intake is a risk factor for diseases such as systemic arterial hypertension, stroke, left ventricular hypertrophy, and chronic kidney disease (CKD). Objective: to evaluate the correlation between estimated sodium intake by dietary intake and 24-hour urinary excretion in patients with non-dialysis CKD. Material and Methods: a cross-sectional study with 151 individuals. Demographic, socioeconomic, clinical and lifestyle data were evaluated. Sodium was dosed in 24-hour urine and estimated by 24-hour Food Recall (R24h). To evaluate the association between demographic, anthropometric, nutritional and laboratory variables with sodium excretion in 24-hour urine, variance analysis (ANOVA) or Kruskal-Wallis test were used. The correlation between 24-hour urinary sodium excretion and dietary sodium intake was performed by Spearman's correlation coefficient. Results: mean age was 60.8 ± 11.8 years, 51.7 % were women. Hypertensive patients, 88.9 %; diabetics, 45.0 %; and 39.1 % were in stage 3B of CKD. Median sodium excretion in 24-hour urine was 112.2 mmol/L and R24h intake was 833.8 mg/day. Individuals belonging to the highest tertile of sodium excretion (T3) presented lower PTH values, and those with lower tertile (T1), higher serum HDL-c levels (p < 0.05). There was no statistical correlation between dietary sodium intake and 24-hour urine excretion (p-value = 0.241). Conclusion: the non-correlation between sodium obtained by 24-hour urinary excretion and dietary intake demonstrates the fragility of the estimation of sodium excretion through the dietary survey. Objetivo: evaluar la correlación entre la ingesta estimada de sodio y la excreción urinaria de 24 horas en pacientes con ERC sin diálisis.Métodos: estudio transversal con 151 individuos. Se evaluaron datos demográficos, socioeconómicos, clínicos y de estilo de vida. El sodio se cuantificó en orina de 24 horas y se estimó en Food Recall (R24h) de 24 horas. La mediana de excreción de sodio en orina de 24 horas fue de 112,2 mmol/L y la ingesta de R24h fue de 833,8 mg/día. Los individuos pertenecientes al tercil más alto de excreción de sodio (T3) presentaron valores de PTH más bajos y aquellos con niveles más bajos de tercil (T1), mayores niveles séricos de HDL-c (p < 0,05). No hubo correlación estadística entre la ingesta de sodio en la dieta y la excreción de orina durante 24 horas (valor p = 0,241). ABSTRACT Objective To analyze the clinical and sociodemographic characteristics and survival of individuals with severe acute respiratory syndrome due to COVID-19 according to the COVID-19 vaccination schedule, Brazil, 2021-2022. Methods This was a cohort study based on data from the Influenza Epidemiological Surveillance Information System; the Kaplan-Meier and Survival Tree methods were used to analyze survival. Results Among the 559,866 hospitalized cases, a higher proportion of vaccinated individuals was found among female (15.0%), elderly people aged ≥ 80 (34.5%), people from the Southeast region (15.7%), those who did not undergo respiratory support (21.2%) and those who did progress to death (15.2%); the survival curve showed that risk of death for unvaccinated individuals was higher in all age groups (p-value < 0.001); elderly people aged ≥ 80, who did not undergo mechanical ventilation and who had a booster dose had lower risk when compared to their peers who had two doses or were unvaccinated (hazard ratio = 0.64; 95%CI 0.62;0.67). Conclusion Lowest risk of death was found in vaccinated individuals, especially those who had two doses or a booster dose as well.


RESUMEN Objetivo Analizar las características clínicas, sociodemográficas y supervivencia de individuos con síndrome respiratoria aguda grave por COVID-19, según el esquema de vacunación contra COVID-19, Brasil, 2021-2022. Métodos Estudio de cohorte con datos del Sistema de Informação de Vigilância Epidemiológica de la Gripe; se utilizó el método de Kaplan-Meier y el árbol de supervivencia para analizar la supervivencia. Resultados Entre los 559.866 casos hospitalizados, se observó una mayor proporción de vacunados entre el sexo femenino (15,0%), ancianos ≥ 80 (34,5%), región Sudeste (15,7%), quienes no recibieron soporte ventilatorio (21,2%) y no fallecidos (15,2%); la curva de supervivencia mostró que los no inmunizados presentaron un mayor riesgo de óbito en todos los grupos de edad (p-valor < 0,001); los ancianos ≥ 80, que no recibieron ventilación mecánica, con dosis de refuerzo, tienen un menor riesgo en comparación con sus pares con dos dosis o no vacunados (HR = 0,64; IC95% 0,62;0,67). Conclusión El menor riesgo de óbito se observó en individuos vacunados, especialmente aquellos con dos dosis o refuerzo.


RESUMO Objetivo Analisar as características clínicas e sociodemográficas e a sobrevida de indivíduos com síndrome respiratória aguda grave por covid-19, segundo esquema vacinal contra covid-19, Brasil, 2021-2022. Métodos Estudo de coorte, com dados do Sistema de Informação de Vigilância Epidemiológica da Gripe; métodos de Kaplan-Meier e árvore de sobrevivência foram utilizados para analisar a sobrevida. Resultados Dos 559.866 casos internados, observou-se maior proporção de vacinados entre sexo feminino (15,0%), idosos ≥ 80 anos (34,5%), na região Sudeste (15,7%), entre os que não receberam suporte ventilatório (21,2%) e os não evoluídos a óbito (15,2%); na curva de sobrevida, não imunizados apresentaram maior risco de óbito, independentemente da faixa etária (p-valor < 0,001); idosos que não realizaram ventilação mecânica, com dose de reforço, apresentaram menor risco, comparados a seus pares com duas doses ou não imunizados (hazard ratio = 0,64; IC95% 0,62;0,67). Conclusão Observou-se menor risco de ocorrência de óbito nos indivíduos vacinados, especialmente com duas doses ou reforço.

ABSTRACT BACKGROUND: Multimorbidity can influence intensive care unit (ICU) admissions and deaths due to coronavirus disease (COVID-19). OBJECTIVE: To analyze the association between multimorbidity, ICU admissions, and deaths due to COVID-19 in Brazil. DESIGN AND SETTING: This cross-sectional study was conducted using data from patients with severe acute respiratory syndrome (SARS) due to COVID-19 recorded in the Influenza Epidemiological Surveillance Information System (SIVEP-Gripe) in 2020. METHODS: Descriptive and stratified analyses of multimorbidity were performed based on sociodemographic, ventilatory support, and diagnostic variables. Poisson regression was used to estimate the prevalence ratios. RESULTS: We identified 671,593 cases of SARS caused by COVID-19, of which 62.4% had at least one morbidity. Multimorbidity was associated with male sex, age 60-70 and ≥ 80 years, brown and black skin color, elementary education and high school, ventilatory support, and altered radiologic exams. Moreover, all regions of the country and altered computed tomography due to COVID-19 or other diseases were associated with death; only the northeast region and higher education were associated with ICU admission. CONCLUSION: Our results showed an association between multimorbidity, ICU admission, and death in COVID-19 patients in Brazil.

RESUMO OBJETIVO Determinar os pontos de corte da circunferência do pescoço (CP) e da relação cintura-estatura (RCEst) para a predição da obesidade e do risco cardiovascular em adolescentes. MÉTODOS Estudo transversal desenvolvido com uma subamostra de 634 adolescentes de 18 e 19 anos de idade pertencentes à terceira fase da coorte "RPS" (Ribeirão Preto, Pelotas e São Luís) realizada em 2016. Identificou-se a área sob a curva ROC (AUC) para avaliar a capacidade preditiva da CP e RCE em relação ao percentual de gordura corporal (%GC), obtido pela pletismografia por deslocamento de ar (PDA), e do risco cardiovascular estimado pelo Phatobiological Determinants of Atherosclerosis in Youth (PDAY). RESULTADOS A prevalência de obesidade pelo %GC foi de 7,6% no sexo masculino e 39,4% no sexo feminino (p-valor < 0,001) e o alto risco para PDAY foi de 13,8% e 10,9%, respectivamente. Para a RCEst, o ponto de corte identificado foi de 0,50 para ambos os sexos e as AUC para a predição da obesidade e do alto risco segundo o PDAY foram de 0,90 (IC95% 0,80-0,99) e 0,73 (IC95% 0,63-0,82), respectivamente, para o sexo masculino; e de 0,87 (IC95% 0,83-0,90) e 0,55 (IC95% 0,45-0,65), respectivamente, para o sexo feminino. CONCLUSÃO RCEst e CP como bons discriminadores para avaliar a obesidade e risco cardiovascular em adolescentes, especialmente no sexo masculino.
